Pada mulanya, ianya dikendalikan oleh seorang Ketua Jururawat dengan dibantu oleh seorang Penyelia Jururawat.<\/p>\n\n\n\n<p>Bermula tahun 2018, hanya seorang Pegawai Perubatan ditempatkan di unit ini sepenuh masa hingga Jun 2022. Dengan kerjasama daripada Pengurusan Tertinggi HSA dan semua jabatan\/unit, unit ini bertindak dalam <strong>memantau dan menyelaras aktiviti kualiti<\/strong> seperti:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Hospital Performance Indicators For Accountability (HPIA) \ud83d\udcca<\/li>\n\n\n\n<li>Clinical Services Quality Indicators (CliSQI) \ud83d\udcc8<\/li>\n\n\n\n<li>KPI (Key Performance Indicator) \ud83c\udfaf<\/li>\n\n\n\n<li>Peri-Operative Mortality Review (POMR) \ud83c\udfe5<\/li>\n\n\n\n<li>Morbidity And Mortality Review (MMR) \u2695\ufe0f<\/li>\n\n\n\n<li>Pelaporan Insiden berkaitan Keselamatan Pesakit \ud83d\udee1\ufe0f<\/li>\n\n\n\n<li>Patient Safety Goals \ud83c\udfc6<\/li>\n\n\n\n<li>Akreditasi \ud83d\udcdd<\/li>\n\n\n\n<li>Privileging \ud83d\udc69\u200d\u2695\ufe0f<\/li>\n\n\n\n<li>Kajian Kepuasan Pelanggan \ud83d\udcac<\/li>\n\n\n\n<li>Audit Klinikal \ud83e\uddfe<\/li>\n\n\n\n<li>Dan sebagainya<\/li>\n<\/ul>\n\n\n\n<p>Unit ini memainkan peranan penting dalam memastikan <strong>kualiti penjagaan kesihatan dan keselamatan pesakit di hospital<\/strong> sentiasa dipantau dan diperbaiki.<\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h2 class=\"wp-block-heading\"><strong>VISI<\/strong> \ud83c\udf1f<\/h2>\n\n\n\n<p>Menjadi pusat kecemerlangan bagi aktiviti <strong>peningkatan kualiti dan keselamatan pesakit<\/strong> di Hospital Sri Aman.<\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h2 class=\"wp-block-heading\"><strong>MISI<\/strong> \ud83c\udfaf<\/h2>\n\n\n\n<p>Menerapkan sikap yang positif dan komitmen yang teguh terhadap aktiviti peningkatan kualiti, sambil menggunakan pelbagai inisiatif kualiti sebagai <strong>alat pengukur<\/strong> bagi meningkatkan tahap perkhidmatan penjagaan kesihatan dan keselamatan pesakit selaras dengan keperluan Program Peningkatan Kualiti Penjagaan Perubatan.<\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h2 class=\"wp-block-heading\"><strong>OBJEKTIF<\/strong> \ud83d\udee1\ufe0f<\/h2>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Bertindak sebagai unit yang <strong>menyelaras, memantau, dan melaporkan aktiviti-aktiviti kualiti<\/strong> di peringkat hospital.<\/li>\n\n\n\n<li>Memastikan aktiviti-aktiviti kualiti di peringkat hospital <strong>dijalankan dan dipantau<\/strong>.<\/li>\n\n\n\n<li>Menganalisa pencapaian aktiviti peningkatan kualiti mengikut standard yang ditetapkan.<\/li>\n\n\n\n<li>Mengenalpasti kekurangan atau penambahbaikan yang perlu dilaksanakan dari segi perkhidmatan penjagaan kesihatan dan keselamatan pesakit serta mencari langkah penambahbaikan demi memastikan kualiti perkhidmatan kesihatan yang cekap dan efisyen.<\/li>\n<\/ul>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h2 class=\"wp-block-heading\"><strong>SKOP PERKHIDMATAN<\/strong> \ud83d\udccb\ud83d\udcbc<\/h2>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Mengumpul data, menyemak dan menghantar pelaporan pencapaian\/data KPI (Key Performance Indicator), HPIA (Hospital Performance Indicator for Accountability), CliSQI (Clinical Services Quality Indicators), Aktiviti Performance Audit serta pelaporan indikator lain yang berkaitan.<\/li>\n\n\n\n<li>Mengumpul dan menyemak pelaporan SIQ (Shortfall in Quality) dan laporan-laporan lain di peringkat hospital yang berkaitan dengan HPIA dan CliSQI, dan menghantar laporan tersebut kepada Jabatan Kesihatan Negeri Sarawak (JKNS).<\/li>\n\n\n\n<li>Mengadakan <strong>lawatan pemantauan berkaitan aktiviti kualiti (audit)<\/strong> termasuk audit dalaman HPIA dan CliSQI di peringkat hospital dan mengikuti audit luaran apabila dipanggil.<\/li>\n\n\n\n<li>Menyelaras dan mengendalikan <strong>Mesyuarat Jawatankuasa HPIA dan KPI<\/strong>.<\/li>\n\n\n\n<li>Memantau maklumbalas\/laporan aktiviti audit klinikal, inovasi, QAP (Quality Assurance Project) dan penyelidikan.<\/li>\n\n\n\n<li>Menyelaras dan mengendalikan <strong>Mesyuarat Jawatankuasa Audit Klinikal<\/strong> peringkat hospital.<\/li>\n\n\n\n<li>Mengelola kursus\/latihan\/bengkel berkaitan aktiviti inisiatif\/peningkatan kualiti.<\/li>\n\n\n\n<li>Mengumpul data dan menghantar pelaporan pencapaian <strong>Piagam Pelanggan Teras KKM<\/strong> kepada JKNS.<\/li>\n\n\n\n<li>Menerima laporan dan menyelaras laporan insiden berkaitan keselamatan pesakit; menghantar laporan minimal report \/ Root Cause Analysis (RCA) \/ Multiple Incident Root Cause Analysis (MIRCA) ke JKN dan memastikan pelaksanaan langkah penambahbaikan.<\/li>\n\n\n\n<li>Mengumpul data dan menghantar pelaporan pencapaian <strong>Patient Safety Goals<\/strong> kepada JKNS.<\/li>\n\n\n\n<li>Menyelaras dan mengendalikan <strong>Mesyuarat Jawatankuasa Keselamatan Pesakit<\/strong> di peringkat hospital.<\/li>\n\n\n\n<li>Menyelaras, mengendalikan mesyuarat dan menyediakan laporan berkaitan <strong>Morbidity and Mortality Review (MMR)<\/strong>.<\/li>\n\n\n\n<li>Menerima laporan kes, menyelaras dan menghantar data <strong>Perioperative Mortality Review (POMR)<\/strong>; serta menyelaras mesyuarat berkaitan POMR di peringkat hospital.<\/li>\n\n\n\n<li>Menyelaras aktiviti latihan akreditasi di peringkat hospital.<\/li>\n\n\n\n<li>Menyelaras dan mengendalikan <strong>Mesyuarat Jawatankuasa Akreditasi<\/strong>.<\/li>\n\n\n\n<li>Menyelaras dan mengendalikan <strong>Mesyuarat Jawatankuasa Penasihat Perubatan \/ Medical Advisory Committee (MAC)<\/strong>.<\/li>\n\n\n\n<li>Mengumpul permohonan baru dan pembaharuan <strong>Sijil Privileging staf<\/strong> Hospital Sri Aman dan mengendalikan Mesyuarat Jawatankuasa Privileging Kluster Sarawak Tenggara.<\/li>\n\n\n\n<li>Menyelaras dan menganalisa <strong>Kajian Kepuasan Pelanggan (PSQ-18)<\/strong> di peringkat hospital.<\/li>\n<\/ul>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h2 class=\"wp-block-heading\"><strong>KESIMPULAN<\/strong> \u2705<\/h2>\n\n\n\n<p>Unit Kualiti Hospital Sri Aman memainkan peranan penting dalam <strong>meningkatkan standard kualiti dan keselamatan pesakit<\/strong> melalui pemantauan, audit, laporan, dan koordinasi pelbagai aktiviti kualiti.
