PERMOHONAN LATIHAN INDUSTRI/LATIHAN SANGKUTAN DI PUSAT JANTUNG SARAWAK

APPLICATION FOR ELECTIVE POSTING IN PUSAT JANTUNG SARAWAK

 

BAGI WARGANEGARA MALAYSIA

Sebarang permohonan untuk Latihan industri/Latihan sangkutan di PJS adalah menerusi Permohonan Bertulis. Latihan Industri/Latihan sangkutan dalam mana-mana bidang atau jurusan adalah seperti berikut :

  • Latihan industri/sangkutan melebihi satu (1) bulan perlu memohon menerusi

Setelah kelulusan diterima, surat kelulusan boleh dikemukakan kepada PJS menerusi Faks : 082-668001 atau e-mel kepada pjs@moh.gov.my.

  • Latihan industri/sangkutan kurang satu (1) bulan perlu memohon menerusi

BAGI WARGANEGARA ASING (FOR FOREIGNER STUDENTS)

Any foreign student entry into Sarawak, the normal procedure is that he/she is required to apply a Visit Pass from the Immigration Department and the entry clearance from the Police Department and the State Government.

For the above purpose, the following documents are required:-

  1. To return through Jabatan Kesihatan Negeri Sarawak office a duly completed Immigration Visit Pass – Immigration Form (Immigration Regulation 1963 Imm.12-Pin. 3/68)
  2. Three (3) copies of passport size photograph
  3. A photocopy of “International Passport” showing your personal particulars.

All application for elective posting to Pusat Jantung Sarawak must be address to:

You are kindly reminded that the Immigration Department will give you a Visit Pass of one month only and you need to pay RM90.00 (Malaysian Ringgit Ninety) once only. For further extension of your stay, you are allowed a maximum period of 3 months. If you need to extend your stay it can be done with the Immigration Department through Sarawak Health Department office. Please do not send any cash or give advance payment because the fees will be collected by the Immigration Department when you come in person to apply for such extension.
