Latihan Industri\/Latihan sangkutan dalam mana-mana bidang atau jurusan adalah seperti berikut :<\/p>\n<ul>\n<li>Latihan industri\/sangkutan perlu memohon menerusi<\/li>\n<\/ul>\n<p style=\"padding-left: 90px;\"><strong>Pengarah,<\/strong><\/p>\n<p style=\"padding-left: 90px;\">Jabatan Kesihatan Negeri Sarawak,<\/p>\n<p style=\"padding-left: 90px;\">Jalan Diplomatik, Off Jalan Bako,<\/p>\n<p style=\"padding-left: 90px;\">93050 Kuching, Sarawak<\/p>\n<p style=\"padding-left: 60px;\">\u00a0atau e-mel kepada myera@moh.gov.my.<\/p>\n<p>&nbsp;<\/p>\n<p><strong>BAGI WARGANEGARA ASING (<em>FOR FOREIGNER STUDENTS<\/em>)<\/strong><\/p>\n<p>Any foreign student entry into Sarawak, the normal procedure is that he\/she is required to apply a Visit Pass from the Immigration Department and the entry clearance from the Police Department\u00a0and the State Government.<\/p>\n<p>For the above purpose, the following documents are required:-<\/p>\n<ol>\n<li>To return through Jabatan Kesihatan Negeri Sarawak office a duly completed Immigration Visit Pass \u2013 Immigration Form (Immigration Regulation 1963 Imm.12-Pin. 3\/68)<\/li>\n<li>Three (3) copies of passport size photograph<\/li>\n<li>A photocopy of \u201cInternational Passport\u201d showing your personal particulars.<\/li>\n<\/ol>\n<p>All application for elective posting to Pusat Jantung Sarawak must be address to:<\/p>\n<p style=\"text-align: center;\"><strong>Pengarah,<\/strong><br \/>\n<strong>Jabatan Kesihatan Negeri Sarawak,<\/strong><br \/>\n<strong>Jalan Diplomatik, Off Jalan Bako,<\/strong><br \/>\n<strong>93050, Kuching, Sarawak.<\/strong><\/p>\n<p>&nbsp;<\/p>\n<p><strong>*[REQUIREMENTS (a), (b) and (c) ARE NOT APPLICABLE FOR MALAYSIAN STUDENT]<\/strong><\/p>\n<p>You are kindly reminded that the Immigration Department will give you a Visit Pass of one month only and you need to pay RM90.00 (Malaysian Ringgit Ninety) once only. For further extension of your stay, you are allowed a maximum period of 3 months. If you need to extend your stay it can be done with the Immigration Department through Sarawak Health Department office.
