Jabatan Perubatan Am
Introduction
The Medical Department in 2023 has managed to maintain the quality of care both in the inpatient and outpatient setting by whilst preserving the transition to the endemic period for COVID 19. The outpatient clinic remains at the 1st floor of the specialist clinic block (Pintu K), operating from Mondays to Fridays, 8am – 5pm and encompasses the Medical Outpatient Clinic, Post Discharge Clinic, New Case Clinic, INR Clinic and Heart Failure Clinic. Medical inpatient services are delivered mainly in Med 3, Med 4, Med 5, Female Medical Ward and Male Medical Ward. COVID-19 admission flow and treatment took a big change in 2023 with the emphasis on admitting only those in the severe category or those that requires medical attention for non-COVID related issues. They would be treated in the isolation rooms of Female Medical Ward and the Infectious Diseases Ward with a clear algorithm set for managing surge of patients if the need arises. Medical patients as per previous years are still scattered throughout the hospital lodged in non-medical wards as well as the Emergency Department.
Besides the General Medicine team, the Medical Department is also comprised of various subspecialty units such as the respiratory unit, neurology unit, rheumatology unit, nephrology unit, endocrinology unit, gastroenterology and hepatology unit and geriatric medicine unit. All subspecialty units are located in Sarawak General Hospital except the geriatric medicine unit who resides in Sarawak Heart Centre. Geriatric clinic services are however rendered in Sarawak General Hospital itself. All units within the department shares the same facilities and manpower.
Department Objectives
a) To provide continuous efficient, holistic, evidence-based services for all patients while adhering to the standard operating procedures.
b) To strive to provide appropriate training where needed to healthcare workers of the department with the aim of improving medical and nursing care.
Services Provided
The main emphasis of service provided by the General Medical Team would be the delivery of quality comprehensive and holistic medical care from diagnosis to treatment of patients with various and often complex medical conditions. As the state tertiary hospital with many subspecialities, the General Medical Team plays the role of the anchor, to plan and coordinate an integrated care plan for those with complex health issues that require input from the respective subspecialities and to assist in continuity of care thereafter.
In 2023, the main bulk of the challenge was still in caring for the increasing number of stranded medical patients in the Emergency Department and those lodged in non-medical wards. However, 2023 marked the year whereby a physician was stationed in Hospital Bau to help expand services to decongest Sarawak General Hospital. The team was expected to deliver appropriate clinical management as well as to help follow up patients to further decongest the medical outpatient department. Regular CMEs were also held in Hospital Bau to enhance the knowledge of resident medical officers there to help cope with the management of more complex pathologies.
Besides that, regular updates pertaining to cluster hospital step down was still ongoing with further enhancement by the Emergency Department with regards to the ambulance booking services being done via an online method. Cluster hospital stepdown is still slow owing to the lack of interest from the patient or family members but nevertheless, the medical team will continue to persevere with this for the years to come. The most successful stepdown so far would be the transfer to the geriatric ward in Sarawak Heart Centre. With a group online to refer and discuss cases, the Geriatric Team would take over cases from the wards in Sarawak General Hospital for further care to decongest the bed situation.
Outpatient clinic services in 2023 ran at full capacity with the usual medical outpatient clinics on Tuesday and Thursday taking the bulk of the workload. Heart failure clinic continued to see new cases while deescalating stable ones to community clinic or medical outpatient clinic. Medical officers are still stationed to call defaulters to enquire regarding their condition and for continuity of medication supply. INR clinic was more seamless in 2023 due to point of care INR monitoring to cut down waiting time for missing investigation or from tracing results. Towards the end of the year, as per instructions from the ministry, administrative affairs of the clinic was gradually handed over to the newly recruited medical assistants while the existing nursing staff of the clinic would be transferred out to the ward or other various units throughout the hospital.

Unit Neurologi
Introduction
The Neurology Department of Sarawak General Hospital was set up in 2011 to provide comprehensive neurological service to the citizens of Sarawak. Neurology diseases are estimated to contribute 25% of total medical ward admission in any general hospital. Most states in Malaysian had established neurology service. However, Neurology service in Sarawak occurs via sporadic visiting neurologist from Hospital Kuala Lumpur. Neurological conditions like stroke contribute to major morbidity and mortality. Recent advancement in the understanding of neurological disorder and introduction of thrombolysis treatment in acute stroke and acute neurological conditions and also rehabilitation for patient with neurological disorder had led to better outcome for patients. It is vital to develop the neurology services with trained personal involving multidisciplinary treatment teams with state of the art equipment in order to provide quality management for patient. The geographical location of Sarawak necessitates the development of its own neurology service with the necessary facilities and to manage neurologic condition including acute stroke. It is logistically almost impossible to refer every acute stroke or sick patient to peninsular Malaysian for further management. By setting up the Neurology Unit in Sarawak, we can improve the health care system in the state. In long term, it is also a more economically away of providing a continuous and comprehensive service to the people of Sarawak and cutting the expanses to import service from other states or to send patient to other state to receive treatment. More, ever we could also provide training to the local clinical and hence encourage more researches and opportunities in future.
Objectives
The objective of setting up a full Neurology Unit in Sarawak General Hospital:
- To provide full range of high level and sophisticated neurodiagnostik test in helping the neurologist for an accurate diagnosis, treatment and management of all neurological disorder.
- To advance the field of neurology in the State of Sarawak.

Unit Rekod Perubatan
Services
Medical records management refers to a system of procedures and protocols responsible for governing patient information throughout the entirety of the data lifecycle. From the moment a patient record is created, it must be appropriately stored, secured, and maintained. After it has been retained for the necessary amount of time (its retention period), the record must be properly destroyed. There is a complex set of rules and regulations regarding medical records management, and for good reason. When health records are mismanaged, patients are put at risk. In fact good quality medical records are an essential component of safe and effective healthcare. Good medical records, whether electronic or handwritten are essential for the continuity of care of your patients. For health professionals, good medical records are vital for defending a complaint or clinical negligence claim; they provide a window on the clinical judgment being exercised at the time. The presence of a complete, up-to-date and accurate medical record can make all the difference to the outcome.
In today’s rapidly changing healthcare field, medical records management can be demanding. However, there are steps that organizations can take to protect patient privacy and maintain compliance. Successful medical records management programs engage the entire organization. In order to efficiently monitor patient records from creation through destruction, organizations need a comprehensive taxonomy and indexing system that covers every type of record handled. This assures orderly filing and tracking of records while making searching more efficient and time saving. In this hospital and in most public hospitals in Malaysia the Modified Terminal Digit Filing system (MTDFS) is used to file patient medical records. Indexing on the records are done by the Medical Records clerks based on patients Identity number namely the Malaysian I.C, Passport, Birth certificates and other relevant and valid identity document. Where these documents are not available the admission registration number is used (R/N).
